I don't see the enablement of SASL in the MTA in your CFLAGS.
I use this (under FreeBSD):

SENDMAIL_CFLAGS+=       -I/usr/local/include/sasl
SENDMAIL_CFLAGS+=       -DSASL
SENDMAIL_LDFLAGS+=      -L/usr/local/lib
SENDMAIL_LDADD+=        -lsasl2

Note the -DSASL.
